Liberty Manor, an assisted living facility, failed to properly supervise a resident with a traumatic brain injury who required 24-hour-a-day care and did not train its staff to understand and meet his care needs. The resident died, at the age of 36, from complications associated with eating foreign objects. At autopsy, the medical examiner found numerous plastics bags, ketchup packets, paper towels and candy wrappers in his stomach and small intestines.
